Updated PASS Form

Social Security has updated the Plan to Achieve Self-Support (PASS) application. The new form is streamlined, uses plain language, and is easier to complete.

You can access the application through the http://SocialSecurity.gov website: https://www.ssa.gov/forms/ssa-545.pdf. Along with the updated form, is an updated policy reference for completing the form: https://secure.ssa.gov/apps10/poms.nsf/lnx/0500870023 (SI 00870.023 Completion of Plan to Achieve Self-Support (PASS) Application Form SSA-545-BK).

The PASS work incentive allows individuals to set aside income and/or resources for a specified period so they can pursue a work goal that will ultimately reduce or eliminate the Social Security Income or Social Security Disability benefit they currently receive.
